General Questions

  • Therapy is a collaborative process where you and your provider work together to achieve your goals. This involves following a defined, evidence-based process where both you and your provider have specific rights and responsibilities.

  • Yes, participation in therapy is voluntary. You can choose to stop at any time. Once you achieve your goals, you will review your progress, identify supports to maintain it, and discuss how to return to therapy if needed.

  • At your intake appointment, you and your provider will discuss what to expect throughout therapy, including the type of therapy, the length of treatment, and the potential risks and benefits.

  • Yes, if your provider is practicing under the supervision of another professional, they will inform you about their supervision and provide the name of the supervising professional.

Intake & Privacy

  • Living Wholehearted utilizes HIPAA secure record-keeping, text/email/fax communication, and maintains HIPAA-compliant practices to ensure your privacy.

  • When scheduling an intake appointment with one of the counselors at Living Wholehearted, they will ask for the client’s email and date of birth to send an invite to our TherapyNotes Client Portal.

  • Through the TherapyNotes Client Portal, you can access and complete all intake forms, and e-sign them on any device. However, a PC/MAC is recommended to fill out the intake forms. Alternatively, intake forms can be emailed to you to complete and return to your counselor.

  • If you are unable to access the Portal or prefer to print your own paper copies of the forms and hand-write your answers, please email your counselor.

  • All intake forms must be filled out and submitted prior to the appointment time to ensure they have been reviewed by the clinician. This allows the intake appointment time to be dedicated to you, rather than paperwork.

  • Please note that when using paper forms, privacy cannot be secured outside of the Living Wholehearted counseling offices. We recommend using the TherapyNotes Client Portalfor secure and convenient form submission.

  • The TherapyNotes Client Portal is an online platform where you can access and complete all intake forms, and e-sign them on any device. It is also used for teletherapy sessions. A PC/MAC is recommended for filling out the forms.

  • Yes. Superbills are provided upon request for services paid in full at time of service. A superbill is an itemized list of services provided to a counseling client that acts to an insurance company as a receipt for counseling. Depending on your insurance, you might be eligible to submit superbills to your out-of-network plan(s) for monetary reimbursement.  Please contact your insurance company for further details regarding your specific plan(s) and the process to submit a superbill to your insurance provider.

    Please note that your insurance may or may not cover/reimburse for the services provided at Living Wholehearted. Payment is due in full at time of service.


    Please check with your insurance carrier to discuss the benefits, limitations and procedures of your plan PRIOR to your visit because you remain responsible for any unpaid balances.

  • You get to choose a counselor with which you have the best fit! Depending on their current availability, each of the counselors at Living Wholehearted offer FREE 10-minute calls to help you decide who you want to work with.

    Paying in full at time of service, rather than utilizing insurance benefits provides you the greatest freedom and choice over your care, including: length and duration of visits, modalities used,  selection of practitioner, privacy of your PHI and medical records, and confidentiality of diagnosis and treatment plan. 

    Managed care plans may deny payment or reduce benefits if your care is obtained outside of their network or outside of their authorization (pre-approval).  As well, your number of sessions or treatment protocols may be limited. 

    Insurance companies require counselors to submit your personal information, including but not limited to: diagnosis, type of treatment, dates of service, copies of chart notes, justifications for treatment, progress reports, and financial information.

    Once information is released to an insurance company, Living Wholehearted has no control over who is able to access that information. Therefore, if you choose to have your insurance cover a portion of your treatment cost, you will be waiving your right to confidentiality as related to the information your insurance carrier requires.

  • Yes! You are entitled to receive a Good Faith Estimate (GFE) of what the anticipated charges might be for psychotherapy services provided to you. While it is not possible for a psychotherapist to know, in advance, how many psychotherapy sessions may be necessary or appropriate for a given person, the GFE provides an estimate of the cost of services provided. Your total cost of services will depend upon the number of psychotherapy sessions you attend, your individual circumstances, and the type and amount of services that are provided to you. The GFE is not a contract and does not obligate you to obtain any services from the provider(s) listed, nor does it include any services rendered to you that are not identified here.


    A Good Faith Estimate is not intended to serve as a recommendation for treatment or a prediction that you may need to attend a specified number of psychotherapy visits. The number of visits that are appropriate in your case, and the estimated cost for those services, depends on your needs and what you agree to in consultation with your therapist.  You are entitled to disagree with any recommendations made to you concerning your treatment and you may discontinue treatment at any time. You have a right to initiate a dispute resolution process if the actual amount charged to you substantially exceeds the estimated charges stated in your Good Faith Estimate (which means $400 or more beyond the estimated charges).


    You are encouraged to speak with your provider at any time about any questions you may have regarding your treatment plan, or the information provided to you in a Good Faith Estimate.

  • Yes! Scholarships are awarded based on income, family size/dependents, as well as special extenuating circumstances. Scholarships are approved for a set number of appointments and can be reapplied for as needed.


    Please note: Any payment plan and/or fee reduction may be modified and/or discontinued: should a credit card payment or check be denied or returned from the bank, for no-shows, or late cancellations.


    In the event a card is declined at the time of session, for a reduced fee appointment, and the fee is not paid within 1 calendar week from the time of the appointment, the balance will default to the Provider’s standard fee for the scheduled appointment time.


    Living Wholehearted Scholarship Funds cannot be utilized to pay for cancellation fees.

    Living Wholehearted offers 3 scholarship application options:

    • a donor can write a check/pay with credit card in your name directly to the provider for the full amount of the session (this avenue will waive your right to confidentiality).

    • a donor can access our donor link and submit their donation, making sure to put your name in the subject line (this avenue will waive your right to confidentiality).

    • 3rd party link

  • Depending on the provider, there is a 24 or 48 hour notice required to cancel or reschedule any appointment, or the client will be responsible for paying for the cancelled session in full. Appointments on a Living Wholehearted Fee Reduction Plan, including appointments paid for in part by the Living Wholehearted Scholarship Fund or third-party funding sources, will maintain the full session fee responsible by the client. Living Wholehearted Scholarships cannot be used to pay for late cancellations. Insurance does not cover missed appointments or late cancellation fees.


    Same-day cancellations due to personal/family illness or emergency preventing the client from being able to attend appointments in person will be provided the option to switch to a telehealth appointment or to reschedule within the calendar week based on Provider availability. If clients need to cancel on the same day due to personal or family illness/emergency and cannot switch to teletherapy or reschedule within the calendar week, the client will be charged the full session amount.

  • Payment is due at the time of the session. Cards on file will be run at the time of the session. Cash, check, and cards issued by major credit card companies are all accepted forms of payment. All checks are made out to Living Wholehearted LLC. A $50 fee will be charged for returned checks in addition to any bank charges. Any discount or payment options may be modified and/or limited should your check be returned.


    First appointments are scheduled as  intake appointments which can take between 60-90 minutes to complete. Please review the fees associated with your provider on their individual web page and/or contact them for further details.


    At your intake appointment you will discuss with your Provider how your appointments will be scheduled, regarding their length and frequency.


    The Living Wholehearted Scholarship Fund is available for all clients to apply to. Eligibility is determined based on financial need, number in family and other extenuating circumstances.


    Any payment plan and/or fee reduction may be modified and/or discontinued: should a credit card payment or check be denied or returned from the bank, for no-shows, or late cancellations. If a card is declined at the time of session, for a reduced fee appointment, and the fee is not paid within 1 calendar week from the time of the appointment, the balance will default to the Provider’s standard fee for the scheduled appointment time.


    Phone calls, emails, text messages, other communication, letters, reports, record reviews, assessments, consultations, or case management practices made by the Provider at your request are prorated at the Standard Fee (in 15-minute increments). All payments are due at the time of service. This includes phone calls, emails, text messages or other communication, with parents when child/teen clients are unable to be seen due to extenuating circumstances.


    Three times the Provider’s Standard Fee will be charged for court appearances, preparation for legal testimony, letters, reports, travel, any meetings (in-person or otherwise) for matters related to legal matters, and testimony, etc. for legal matters made by the counselor at your request or the request of your attorney, a judge, or a court office.


    Any appointments canceled with less than 24-hours notice prior to the agreed-upon scheduled appointment time will be charged the full session fee.


    Same day cancellations due to personal/family illness or emergency, preventing the client from being able to attend appointments in person, will be provided the option to switch to a telehealth [video via TherapyNotes Portal] appointment or to reschedule within the calendar week based on Provider availability.
    If clients need to cancel the same-day due to personal or family illness/emergency, and cannot switch to teletherapy or reschedule within the calendar week, the full session fee will be charged.


    Living Wholehearted periodically reviews and adjusts the standard fees and discounts. All current clients will will be informed if/when standard fees are raised.


    Living Wholehearted may use outside professional businesses for billing, administrative assistance and collection purposes. These businesses are under the same HIPAA regulations as Living Wholehearted, in that we both use appropriate safeguards to prevent the misuse or disclosure of your personal health information.
